The following documents are what is needed when you are writing a contract with the builder. The Installed Contract will have the contract with the builder and the Scope of Work Addendum included. You will need a copy of the Scope of Work addendum document to give to the installer with their contract.

When you turn in your contract to the office BEFORE the project starts you will need the following documents:

• Installed Services Contract (with Owner/Builder)- with scope of work filled out
• Installed Package Worksheet
• Estimate / Bid (materials dump)
• Sub-Contractor Agreement from approved S-C (doc not available at this time) use Annual Subcontractor Agreement with sub-contractor responsibility sheet
• Any written bids / estimates by any sub-contractor or supplier

Other documents that may need to be added as the project is under way:

• Installed Change Order Sheet
• Lien Waivers (provided by Tim or myself)
• any other item that is deemed necessary

The links above will take you to a folder that has the documents you will need. If there is not a document that you think is needed please contact me and I will get you what is needed.

Any Guaranteed Pricing to builders MUST include the form in the link above. It states our terms and conditions and protects us, some, from changes and other problems that are consistently arising.

The Evaluation Links tab will include links that you can cut and paste in an email to your builder. It is an evaluation survey about the services we provided to them. This evaluation is in its infancy stages and may change, or evolve, over time. If you have any suggestions about this form please let me know and I will see about making the suggested changes. If there are other types of evaluations you would like to use let me know and I will set you up with an account and help you make all the evaluation forms you need.

The Other tab will have different forms or information from time to time.. I will be adding additional forms or items to this tab that do not belongin any of the other tabs. If you have a suggestion as to what can go in there let me know. Currently the O.S.B adn T.R.I.M. certificates are located there.
